​Occupational Therapy

• Daily Living Skills Training: Helps patients regain independence with bathing, dressing, grooming, toileting, and other everyday activities.

• Home Safety & Fall Prevention: Evaluates the home environment and recommends modifications to improve safety and reduce fall risks.

• Strength & Functional Mobility: Improves upper-body strength, coordination, balance, and functional abilities needed for daily activities.

• Adaptive Equipment & Caregiver Education: Teaches patients and caregivers how to use assistive devices, adaptive techniques, and energy conservation strategies to promote independence at home.

Speech Therapy

Neurological, orthopedic, and traumatic conditions that affect the ability to speak, swallow, problem solve, word

find and other related communication difficulties are addressed by a licensed speech pathologist. This specialized therapist assists the patient in achieving his or her maximum potential relating to communication and oral-motor skills.

Home Health Aide

• Personal Care Assistance: Helps patients with bathing, dressing, grooming, toileting, and other daily hygiene needs.

• Mobility & Safety Support: Assists with walking, transfers, positioning, and fall prevention to maintain patient safety.

• Daily Living Assistance: Provides light housekeeping, meal preparation, and assistance with eating as outlined in the care plan.

• Observation & Reporting: Monitors patients for changes in physical or emotional condition and reports concerns to the supervising nurse.

 

Social Work

• Psychosocial Assessment & Support: Assesses patients’ emotional, social, and financial needs that may affect their health, recovery, and ability to remain safely at home.

• Patient & Family Counseling: Provides short-term emotional support and guidance to help patients and caregivers cope with illness, disability, and challenges related to home care.

• Community Resource Coordination: Connects patients with financial assistance, transportation, meal services, caregiver support, and other community resources.

• Care Planning & Long-Term Support: Collaborates with the home health team to address barriers to care, coordinate additional services, and assist with transitions to appropriate levels of care.
